And my pick for a recent adaptation that I really loved might be controversial (it was with my family!), but I really loved the most recent iteration of Little Women starting Saoirse Ronin and Emma Watson! Little Women was an early love of mine and I really liked this fresh (but faithful!) take on it!
I would have to agree with you that the Eragon adaptation was really bad! It was truly disappointing as I had really enjoyed the book series. Along this line, I found the movie adaptation of Rick Riordan's "The Lightning Thief" equally distasteful.
Additionally, as a reader of the "Divergent" series, I found the movies okay, but I was annoyed with the portrayal of Tris as more accepting of violence than she was in the books. But I suppose you can't have a character that dislikes guns in an action movie!
I think perhaps maybe I'm too picky of a critic, but many of my favorite books/series that have been made into movies don't seem to be that faithful to the characters and the storyline.
My general rule is that the books are almost always better than the movie or TV show!

For a TV show, I do like the adaptation of The Terror by Dan Simmons (the first season) where some of the things changed actually added to it.
My least favorite adaptation is a toss up between so many movies and shows that it is hard to narrow it down. Maybe the Dresden files based off of Jim Butcher's series or the Eragon movie?
