Other: He has to scar running up his back/shoulder blades like a large V, they represent where he got his wings torn off. (For being Fallen.) So he can no longer have wings and fly.

I don't think all have too. I don't know if I may have picked that up from a few stories about Fallen losing their wings... I used it because the idea of 'Fallen' for me means you can no longer fly, and are from forbidden to enter heaven again. & to enter heaven you would need wings to ascend there. (So to speak anyway.)

More often than not. If they've been banished from heaven because of an inexcusable sin or something like that, then yes. But if they ran away or something, then no.

But there are many ways to become a Fallen, so use your judgement as to whether or not your character got to keep his/her wings
