Looks like this was pulled out of Showcase Presents and into standard TPB recently.
Also, references to "Suicide Squad #67" are to a one-shot comic book during Blackest Night; because of the numbering, this would technically be part of Suicide Squad I, otherwise published 1987-1992. (It's collected in Birds of Prey: Dead of Winter.)
Suicide Squad II, from 2001-2002, doesn't seem to be collected at all.
