Living forever, no children, and watching the world turn ugly(also needing to have multiple surgeries)
VS
Living your life to the fullest for the time you have, and die leaving your intellegence to the next generation knowing the world will move on and keep adapting.

Which would you choose?

I wouldn't sign the declaration

I would deffinitally think about it (with my husband if I was married) But I would be on the Not Taking it side. I think a regular, about 70-80 years is enough living. Plus, I wouldn't be looking to having surgery ever other month. EW!
