This book reminds me of one episode of Monk where the wife kept acting normal one second and then crazy the next, and it turns out that her psycho twin sister was masquerading as her the whole time. Seriously, is this a Dr. Jekyll and Mr. Hyde situation? Because Nina has issues that seem far beyond a simple personality disorder.
