The book I'm working on now was inspired by an offhand historical detail about the Jewish library from Rome, seized by the Nazi's and never seen again...

I'm most inspired by learning some striking historical or archaeological fact, like hearing about the Nazi POW camp in Arizona, which inspired The Nazi Skull, or learning that Mongolian throat singing is usually based on the landscape, which sparked The Mongol's Coffin.

Writer's Block, for me, is almost always an indication that I've just moved the book in the wrong direction. If I take a closer look at the last chapter, I'll see that I've missed a better opportunity, or planted the wrong clues. Sometimes, it takes a walk in the woods, or a good foam blaster battle to shake out the right answer!
