Work. Practice. Get feedback and listen to it. When you want to look at being published, don't turn your nose up at small publishers or "indie presses," these will often give new writers a chance where the Big Four and larger companies won't.

Discipline and planning. I've got a plan for the book,with a chapter-by-chapter and even scene-by-scene outline, so I know what's going to happen. If I'm suck, I sit at the laptop and think about what's supposed to happen in the scene I'm trying to write, and I work through it. Doesn't always work, of course, and on days it doesn't, I leave it and do something else.

Probably Middle-Earth. I think I'd just live in the Shire, but I'd want to go and see Rohan, Gondor, the Lonely Mountain, Rivendell, Lorien, and all the wonderful places in Tolkien's world.
