It depends. Sometimes an idea comes in a dream, sometimes, I'll be having a conversation with someone and something they said sounds like an interesting story. Sometimes it's an image I see that has the potential to be more. Of course, the more compelling the idea, the longer it bounces around in my brain, demanding to be turned into something concrete. You know it's a great concept when it comes to life on its own and demands to be put down on paper.

Various ways. Changing my writing location can help, doing another activity while thinking about the story - Agatha Christie would wash dishes - but the best option is usually just sitting down and writing SOMETHING. Even if it's bad, just something to get you from Point A to Point B. And then you can go back and fix it later.

Once there was an author who had many wonderful stories in her head but never made the time to write them down. So the stories died slow deaths in her head as she forgot them over time, and the author decided she'd rather do a job that required no creative insight whatsoever.

Good question, there are a lot of amazing book worlds and there are tons of things I'd want to do in each one. Could I go to a manga world? It'd be interesting to be one of those characters for a day. Ouran Host Club comes to mind at the moment. I'd like to cross-dress with Haruhi and run around saying "damn rich people" for a day.
If not, something from J.D. Robb's "In Death" series. I would pal around with Dallas and Peabody for weeks if I could.

You're going to have days where everything you write sucks. You're also going to have days where you can't think of anything to write. That's okay. The more you write, the better you'll get at it.
