Hey, that makes it two questions! ;)

1) It depends on what "writer's block" entails. In my case, it very seldom stems from a lack of ideas, and has more to do with wanting to do something else more than I want to write. So I basically have to kick myself in the butt, come up with self-imposed deadlines, and tell my so-called Muse to stuff it and get back to work. (I don't like calling that thing a Muse either, come to think of it. It's too dignified a word.)

2) Both, actually. I loosely build a frame around what I consider "the turning-point scenes", and start writing from there. But if I come up with an idea that would make the story better, and/or would allow me to get rid of a plot hole (hello, Slicer, I'm waving at you here), I rearrange things within my "frame". It *has* to be better, though. I'm not doing that with just any idea.(less)
