I guess I am one of the lucky ones, I normally work from home when not travelling for my day job so am used to the isolation that brings.
Now there are a few simple things you should do when you find yourself out of the office environment to make life bearable.
Have a place where you can work without distraction, set up an office if you can or a place where the kids know if you are there you are working.
Keep regular working ours and take breaks.
Don't be afraid to call someone from the office for a chat to keep up contact. They will welcome it as well
You will be much more productive at home and will get through emails and tasks much quicker as you will be less distracted. But its the lack of distraction that will make you feel isolated so use the time to get on the phone or video call a colleague.
Go out in the garden and get some fresh air!
Write a book!
