I've just uploaded a new photo that you can print up and include in your donation box.
This was a great idea from CluckingBell and AstroPheems, members of our group. They pointed out that it can seem odd to send off a shipment of books with nothing more than a return address. That's all that most prisons require, however, it is better to include something to explain where the donation is coming from.
It entirely up to the donor if they wish to include the 'slip' as I call it (to differentiate from a 'donation form,' which some prisons require, though most don't). If you'd like to include it, feel free to print it up off the photos section of our group, modify if you like, make your own, whatever you wish.
