Although a sociopath she is still very intuitive in a twisted kind of way and good at her job. She also picks the clients she can manipulate so they are very damaged people. She probably wouldn't choose anyone she thought could be on to her.
Alex is that good at reading people and responding to what they need. She talks about looking in the mirror to refine her facial expressions to match what she observed other people do. She intuits what a patient needs then responds appropriately until the patient is pulled into her maniacal game.
Normally I would lean towards a combination of the two but given she was able to fool professionals in the psychiatric community, I think she’s that good. Look what she did to poor Bryant, getting him to believe in her innocence without giving the slightest consideration to Kim’s point of view. She had Kim second guessing herself for a time.
