Smashwords - go to your order page. Search for unredeemed books. You can ask that the email be resent to the original recipient - they do not allow, last I checked, that the book be sent to someone else.

Amazon - go to your account, click orders, then digital orders. Any unredeemed books would show up here (you may have Return Unredeemed books to either revoke the gift, or to send it to someone else.

possibly a dumb question, but did you look in your spam folder? I find emails in there all the time.

I had the same problem and looked in every single folder available.
Heather had to PM my the link. There is something hinky going on (actually, business as usual on Amazon.)
That doesn't mean looking in your spam folder is going to hurt ;)
