Diarmid is married, but he doesn't have a wife anymore. Not widowed, but he certainly has lustful thoughts toward Michael.

They arrive at the castle and everyone seems to have an ailment of one type or another. Diarmid is basically desperate for medical help since he has tried everyone else.

Diarmid has a disabled 5 year old niece and he feels he has failed his oath to his dead brother to take care of her. Further, he has a damaged hand that prevents him from doing detail work like surgery. He grabs Michael in the middle of the night and carries her away to treat his niece.

Michaelmas was working at a hospital, but not allowed to do any physician work since the abbot, prioress, and local master physician all prevented her regardless of her medical training and wouldn't grant her a license. She doesn't use her gift since she was tried for heresy in Italy. Further, the abbott here knows about the trial and they seem to be looking for a reason to go after her.
