Prediction: Elspith is forced to let nightmare take over for one of two reasons -
1. Betrayal from someone she knows. My money is on the ex (scorned lover and all that), the miserable step mom, or the friendly physician.
2. It's revealed the twin card has been found and she has to keep it out of the enemy's hands.
