For over 40 years, the level of therapeutic improvement measured in randomized controlled trials and meta-analytic reviews has consistently been a change of about one standard deviation in the mean score on outcome measures, which represents merely a 20 to 25%. Any occurrences of transformational change in outlier cases within an RCT remain invisible and buried under the overall statistical analysis of the data.
