Duke Medical Doctor Eric Westman has been on a keto diet for decades. And this reservation by PhD and MD to endorse a keto diet to treat metabolic disorder has limited the study in-vivo (so to speak). It is the same as with Atkins which Dr Westman followed.

In section 3.11 on Keto diet, this otherwise great book says...
"I’ll add a final word of caution: while ketogenic diets can be an indispensable tool in certain diseases, its use in the presence of mitochondrial disease—at this point—is controversial. So, of all the therapies listed in this book, the one that would seem to require specific expertise in its application would be this diet."
which IMO is unfounded.
