Plastic Surgery Q&A!

Do you recommend Sculptra or HA (hyaluronic acid) filler for temple hollowing? In general, I prefer Sculptra for this area. Hollowing of the temples is a common sign of facial aging, and filling that area out can create better facial harmony. Although HA fillers like Vollure or Restylane work fine in this area, I believe Sculptra is safer, longer-lasting, and may have better results.

I have an overhang over my C-section scar. What are non-invasive options? If you have an area of subcutaneous fat above your C-section scar but no excess skin, then a non-invasive fat reduction procedure like Sculpsure can help thin the fat by approximately 20-25%. However, if you have excess skin hanging over the C-section scar, then only surgery (like a mini tummy tuck) can remove it. This operation can be done in as little as an hour-and-a-half with relatively minimal recovery time.

What is the best way to get rid of brown spots on my face? I recommend combining brightening creams with IPL treatments for the fastest results. How do you handle complications during surgery? Complications are the worst part of being a plastic surgeon. No matter how good a surgeon is, they will get complications. It’s inevitable because the human body can react in unexpected ways. That’s why it’s important to choose a real board-certified plastic surgeon for your plastic surgery. We undergo years of training and continual education so that we can handle virtually any complication that comes our way.

I have mild laxity of my face. Should I use Sculptra or threads? NOT THREADS! Threadlifts create scarring and their results only last about 6 months. Sculptra is a nice biostimulating filler but doesn’t lift or tighten. If you have mild laxity, then I’d recommend a combination of applying a topical retinol every night and RF microneedling treatments such as Morpheus8.

Skin Care Mistakes to Avoid!

We all make mistakes, but with our skin care? We definitely make too many! This week I’ve decided to share with you mistakes you could be making with your skin, and what to do about it.

Using Beef Tallow For Face Cream

Beef tallow is everywhere right now – It’s a cooking favorite for people on the carnivore and ketogenic diet, but it’s also being used in skin care! Some people are making it at home, while others are buying it on Amazon. So what is it?

Beef tallow is basically fat from around the cow’s organs (called the suet) which is slowly heated until the solid fat separates from the other tissue. It’s then cooled and often combined with honey to act as a moisturizer.

Beef tallow contains lots of healthy fats, so it can act as a gentle, natural moisturizer for the body. My issue with beef tallow is that it is comedogenic, so do not apply it to your face. Also make sure to smell it before applying in case it’s become rancid. And finally, there are no true anti-aging benefits to beef tallow, so best to use a moisturizer that contains retinol, bakuchiol, peptides, or growth factors, too!

Going to Bed with Wet Hair

I’m guilty of this sometimes. My wife Amy and I hate to go to bed feeling sweaty, sticky, or dirty, so we will often shower in the evening or before bedtime. The problem with doing this and going to bed with wet hair is that your pillow will absorb the water and stay wet for HOURS.

Warm moistness = bacterial growth (and even fungi!) which can lead to breakouts and inflammation. In fact, many years ago I had a major flareup of my rosacea and discovered that it got better whenever I would sleep in a hotel room for a few nights. But when I eventually returned home the rosacea would flare again.

The rosacea improved big time when I threw my pillow away and got a new one.

Brushing Your Teeth After Applying Skin Care

Are you having breakouts on your chin? If so, then do you brush your teeth AFTER applying skin care? If that’s the case, then it’s possible you’re leaving toothpaste residue on your chin. This can irritate the skin and increase your risk of breakouts.

So please brush your teeth first, then cleanse your face and apply your skin care last!

The Most Exciting Plastic Surgery Innovation in Years – Motiva Breast Implant

What’s the newest and most exciting innovation in my surgical practice in years?

As a plastic surgeon who performs a lot of breast implant surgery, the bane of my existence is capsular contracture – excess scar tissue that can occur around implants. All breast implants are surrounded by a scar tissue pocket. In the vast majority of cases- about 95-98% of them – the scar tissue is thin and doesn’t impact how the result feels afterwards.

But in about 2-5% of cases (up to 10% in some studies), the scar tissue can thicken and harden around the implant, making it look and feel unnaturally firm. This is called a capsular contracture, and if it occurs must be treated surgically.

In my practice I do everything I can to prevent this from happening to my patients, including using an implant insertion funnel, changing gloves, antibiotic rinses, and more. But no matter what I do, I can’t lower the rate of this complication from happening to less than about 2%.

Until now.

Enter the new Motiva breast implant. This implant is the newest, most advanced implant available today. It has a Smooth Silk surface, which studies show reduces the risk of capsular contracture to 0.5%. This is at least four times lower than what we have seen with the traditional smooth silicone and saline implants.

The Motiva implants were FDA approved in late 2024, but have been used around the world for 15 years, with over 4 million Motiva implants placed worldwide. I’ve been using them in my practice for the past 4 months, and so far the results have been great.

Although the jury is still out on whether this implant has a lower rupture rate or risk of Breast Implant Illness (BII), so far my patients have been very happy with them. I’ll keep you up-to-date with new findings!

Habits To AVOID This Winter!

I get it – it can be difficult to acquire and maintain motivation to do the right things for our health when the days are dark and cold during the winter months. I struggle with it too! However, it’s important to avoid falling into the trap of a number of habits that can hurt us in the long run.

Here are three habits to avoid this winter!

Taking Hot Showers – It’s incredibly tempting to put the hot water on full blast when the frigid temps arrive, but showering in hot water is terrible for your skin and causes it to become dry. Instead, take warm showers or baths and don’t stay in for too long. Finish up by moisturizing within two minutes after your shower to lock in hydration for hours!

TIP: Dry Brushing – If you’re finding that your skin is too dry, itchy, and flaky when it’s cold, then consider dry brushing to exfoliate your skin. I recommend using a brush with organic bristles, then brush your body in the direction of the lymphatics. In general, this is from outside to inside. Dry brushing before you shower is a great way to soften your skin and keep it hydrated,

Skipping Your Workouts – This might be a no-brainer, but exercise is significantly important during this time. Think about it – many of us are more sedentary during the cold months, we tend to over-indulge on fats and sweets due to the holidays, and winter time is prime time for germs trying to get the best of our immune systems. Make sure to get in at least 20 minutes of exercise at least every other day to help combat sickness and unwanted weight gain. A treadmill is the best purchase I’ve ever made for my health!

TIP: Download a Workout App! My wife got me into Peloton, first via the app and then with the bike. Access to the Peloton app isn’t exceedingly expensive (currently it’s free for the first month), and it contains tons of yoga, pilates, strength and bootcamp workouts, meditation, and more! It’s a great resource for the winter months when you don’t want to venture outside or to the gym.

Spending Too Much Time Indoors – Unlike during the summer months, it’s sometimes hard to find the motivation to get outdoors when there’s less daylight and harsher weather conditions to contend with during winter. But we need to get outside, even for a short time! The fresh air is great for us, and when we’re cooped up all day, we could be breathing in allergens, dust, germs, and other impurities that could harm our health. When the sun is shining, the Vitamin D is also great for our mood and immune system. So bundle up and get out there!

Avoid These Mistakes When Trying to Look Younger

We all want to look younger, but there are so many ways where we go wrong with cosmetic treatments and skin care! I’m going to give you four of the most common mistakes people make when they want to look younger.

4. Too Much Filler – I’m a big fan of injectable fillers, but it can go wrong! Sometimes a good thing taken too far can be a bad thing. Ever hear of “pillow face?” That’s when too much filler is injected. Now, if you’re interested in facial fillers, I’d recommend starting with the areas that only need a little filler to make a difference, like the front of the cheeks and the cheek bone area. Start with 1/2 to one syringe of filler on each side. This should be enough for you to see a result without being over done.

3. Using Too Many Products On Your Skin – People in Korea caused a skin care revolution by creating multi-step skin care routines that can include up to 15 steps! But you don’t need a long routine to get beautiful, healthy skin. If you do too much to your skin, you can disrupt its microbiome, which can be harmful and may expose you to premature aging. Instead, get back to the basics and do a simple routine! Stick with a cleanser, Vitamin C serum, sunscreen, moisturizer, and anti-aging cream.

2. Being Afraid of Botox – It’s the most popular cosmetic treatment today! Botox temporarily weakens the muscles in the areas in which its injected. It lasts about three to four months. The chance of developing a major problem is very rare, but if you’re nervous about trying it, start with the frown lines or the crow’s feet.

1. Not Paying Attention to Diet and Supplements – What you eat can have a profound impact on your skin! It can contribute to issues like acne, eczema, and rosacea. Nutritional supplements like collagen can help replenish collagen and much of the produce found in grocery stores can help prevent aging as well! What foods specifically? The Second Most Popular Cosmetic Treatment Today!

Injectable fillers are the second most popular cosmetic treatment after Botox. But with the plethora of fillers out there, how are you to know which one is the best for which area? Here are some tips!

Lips: There are a number of great fillers for the lips. Restylane Silk, Restylane Kysse, and Restylane Defyne are great to plump up the lips softly. Juvederm Ultra Plus is a lip filler that can give long-lasting results. For a softer, gentler look, consider Restylane Refyne or Juvederm Volbella.

Nasolabial Folds (smile lines): These are the grooves that extend between the sides of your nose to the corners of the mouth. My preferred fillers for this area are Juvederm Ultra Plus, Juvederm Vollure, Restylane Defyne, and Restylane Lyft.

Cheeks: Adding volume back into the cheeks is one of my favorite things to do. In this area, I prefer Juvederm Voluma, Restylane Lyft, and Restylane Contour. Sculptra is also a nice, longer lasting filler for the cheeks but necessitates multiple treatments.

Marionette Lines: These fine lines and grooves can be challenging to treat. Currently, my favorite fillers for this area are Restylane Refyne and Restylane Defyne.

Jowls and Laxity: This is an area that is being treated more and more. I’ve been using Juvederm Vollure for many patients. although Juvederm Vollux is a new filler specifically FDA approved for the jawline.

If you are considering facial fillers, make sure to be treated in the office of a real plastic surgeon or dermatologist in case you have complications. There are too many people injecting fillers who aren’t properly trained or backed up!

How to Banish Belly Bloat Naturally!

One question that my patients often ask me is, “How can I tell the difference between belly fat and belly bloat?”

This is a pretty simple answer. Belly fat is something that doesn’t really change during the day. Fat doesn’t contract and expand much (with the exception of gaining a little water weight when eating a salty or processed food meal). Belly bloat typically goes up and down dramatically depending on the time of day, activity level, and what you eat.

If you’re suffering from belly bloat, these tips will help!

Reduce the amount of soda pop you drink – Your gut contains trillions of bacteria which are crucial to your body’s gut health, skin health, and even brain health. Excessive amounts of sugar can support unhealthy bacteria to flourish in the gut microbiome. Drinking a lot of soda pop can lead to the excessive growth of unhealthy gut bacteria, and this can cause bloating issues to be worse. Carbonation also makes bloating worse!

Eat more fermented foods – Foods like kim chi, sauerkraut, miso, kombucha, and kefir are filled with good bacteria that can help your gut microbiome. Increasing how much of these foods you eat can bring healthy bacteria to your gut and improve your gut health and bloating. I recommend aiming for eating fermented foods a few times a week if you can, although some holistic health experts recommend eating them every day.

Start a daily probiotic – Taking a daily probiotic with at least 3 billion CFU can be a good way to supplement the good bacteria in your gut. While not a substitute for eating fermented foods, it can help. I’ve talked with many patients who’ve told me that the simple act of taking a daily quality probiotic improved their digestion and bloating.

Drink peppermint tea – This tea is known to improve gut motility to allow food (and gas) to pass through more easily. Drink a cup or two a day to help with belly bloat.

Avoid dairy and gluten – If you’re lactose intolerant, then I recommend avoiding dairy. However, even if you aren’t technically lactose intolerant, then reducing the amount of dairy you eat may improve your bloating. What about gluten? Even if you don’t have Celiac Disease, gluten can cause bloating, cramps, and other symptoms like fatigue and brain fog. So if you have a bloating issue try going off both dairy and gluten for a few weeks and see how you feel. I have a feeling your bloating will improve, as will maybe some other things you might not expect.

Here’s to a flat, non-bloated tummy!

These Are The BEST Cosmetic Treatments To Do Today!

While cosmetic treatments can be done year-round, there are certain treatments that are generally better to have done at specific times of the year – for example, when the sun is less prevalent and it’s more likely you’ll be indoors, or when you can give yourself more time to see results.

Wondering what treatments are the best to have done during the winter months? I’ll fill you in!

IPL – Do you have sun and age spots that you want to get rid of? IPL (Intense Pulsed Light) is the fastest, most effective way to remove them, sometimes in as little as a week! All that with no downtime. It’s great to have done in the winter because sunshine is more scarce and you won’t accidentally reverse the results. Just remember to wear sunblock every day, even in winter!

Laser Hair Removal – There’s no better time to undergo laser hair removal treatments than during the winter when you’re showing less skin and likely not shaving as much as during the summertime. Go ahead and take care of that bikini line or under arm area right now!

SculpSure – This popular non-invasive fat reduction treatment can be done nearly anywhere on the body, and is perfect to do to prepare for bathing suit season or spring break. How Ozempic Makes You Look OLDER!

 

Have you heard of Ozempic? It and its sister drug Wegovy (both have the same active ingredient called semaglutide), are all over the news because so many people are taking them to lose weight. But can it make you look older?

There are reports of a phenomenon called “Ozempic Face.” People who have taken these medications and lost weight are reporting that they look older and gaunt in the face. One person even claims that she doesn’t recognize herself in the mirror anymore! Is there anything to Ozempic Face?

Kind of.

Anytime a person loses weight, they can lose weight (or fat) in the face. It happens to virtually all of us. Combine this with the fact that loss of fat occurs naturally with aging (especially visible in people with more narrow shaped faces like me), and losing weight MIGHT make a person look technically older.

That being said, there are many options if this happens to you. You can undergo fat grafting, where fat is harvested from your hips, thighs, or tummy and injected into the face to replenish the lost volume, or you can consider injections of filler. There are plethora of injectable fillers which are great for adding volume back to the face, including Juvederm Voluma, Juvederm Vollure, Restylane Contour, Restylane Lyft, and Sculptra.

Other than the general loss of facial fat that can occur with ANY significant weight loss, there is no evidence that taking Ozempic or Wegovy will cause your face to unexpectedly change. Ozempic Face is just a clever term some doctors are using to make news.
