I go into a lot of detail about this at my website blog. I've always taken Ray Bradbury's advice when it comes to writing stories. He always said to 'follow the characters,' and that's what I do. I don't outline. I observe my characters.
Books come to me and play out in my mind like a movie. My sci-fi series started from one sentence that came into my head years ago, right before I was getting ready to go to sleep.
