Werner A. Lind R.F., I have moderator responsibilities (mostly along with other people) in ten different Goodreads groups. (I don't have a blog, though!).

Yes, the disclaimer you use is appropriate every time you review a book (in any venue) that you got from the author or publisher for free. However, if you also arranged for a review swap, my understanding is that you're also obligated to disclose that specifically. (Of course, I'm no expert on the legal requirements; I've gotten quite a few free review copies over the years, but have never written a review as part of a swap.)

Just as a technical tip: if you want to reply to something an author writes when using "this question and answer" feature, you don't have to post your reply as a new question, unless you want to. You can use the "comment" link that accompanies the original answer.
