Usually I'll think of a specific scene or dialogue line at an incredibly inopportune moment, write it down in the notes app on my phone and then wait until I have time to sit and write it out. I used to write shorter pieces where this was the whole process, now I'm writing mostly long-form stuff this usually means finding a place for it in the outline.
