Another spy is with her but leaves when they have a disagreement. He returns when he realizes she’s in danger and finds her in the dungeon. If she moves at all, she will die. Probably written in the 1970’s or 1980’s.

Yes, I would say romantic suspense. I remember that she was educated and someone asked her to spend the weekend at the castle to spy on the owner. The other spy, (maybe Hugh?), was there to help but thought she betrayed them by getting romantically involved with the owner, so he left. He found out she never left the castle & went back for her. She had been in the torture device for a while when he found her.
