Bad blood cells are the antagonist because they go against good blood cells and that is the definition of antagonist. Bad blood cells create a problem because they can cause you to get a disease and you can also soon become weak.

The setting in The Book of Blood is based inside your body. Blood is inside of your body and helps you distribute to be healthy. The setting continues to varie in time because no matter how old you get your blood will still remain in the same place and still remaining to help your body become healthier and stronger.
