An antagonist means anything that is creating a conflict for the protagonist. In this book, I would say that the bad blood cells are the antagonist. Bad blood cells are the antagonist because they go against good blood cells and that is the definition of antagonist. Bad blood cells create a problem because they can cause you to get a disease and you can also soon become weak.
