Mistaken identity. A big guy, an actor, suppose to go and intimidate someone. He suffered a heart attack and didn’t make it to the meeting. Reacher is mistaken as the big guy as he is the biggest guy at the coffee shop. Reacher wants to tell the guy he is mistaken.
Of course the bad guys are there to start a fight if this isn’t a Reacher’s story.

The therapist is working for the bad guys and leaking confidential information for the blackmailer. Not that surprise. Many therapists were shallow and couldn’t really help much. Sceptical about talk therapy in general.
