Winston is telling his mistress about how he once seriously contemplated killing his wife. This is the second time that Winston has fantasized about murdering a woman that he hates. He thought about raping and murdering Julia, his current mistress, when he suspected she was spying on him for the thought police. Yet, he didn't want to kill the prostitute. The hate seems to be only directed at women that he fears.

"It was not desirable that proles should have strong political feelings. All that was required of them was a primitive patriotism which could be appealed to whenever it was necessary to make them accept longer working hours or shorter rations." Wow. Is it bad that i immediately thought of not government, but of labor unions when I read this?
