I used to work at a store that had two toilets, one was labeled Ladies the other Gents. They were identical, just mirror images of each other.
There was always a line for the Ladies so the employees just used the Gents. One time a little old lady was waiting in line, asked the lady in front if she minded her skipping the line to use the Gents. The lady went "But you are a lady you can't use the Gents"

The memory of this always makes me laugh.
I can understand maybe not going into a Gents toilet that has a row of urinals because you might get a fright but a one stall, one room toilet, seems a little ridiculous.
When you gotta go you gotta go and let me tell you while backpacking through Europe no one seemed to care if you went into the wrong toilet they just sort of did the nod of "hey we are both in a toilets" acknowledgement thing and went about their day.
