It isn't enough to ask what a clinician could or should have done differently so that he and others may learn for next time. The doctor is often only the final actor in a chain of events that set him or her up to fail. Error experts, therefore, believe that it's the process, not the individuals in it, that requires closer examination and correction. In a sense, they want to industrialize medicine.
