All of this is dependent on the intelligence of the patient. There is an assumption that a patient must have a sufficiently high IQ in order to participate in psychotherapy. I.e., stupid people cannot have their heads shrunk.

Is this potential malpractice? He hasn’t disclosed that he isn’t trained in leading meditation. Also, I don’t like the emphasis on the spiritual. It reminds me of AA, which I am extremely wary of, especially when it is promoted instead of medication.
