Sleep – How much should you get?

When I cover the keys to health, the keys to recovery from a chronic condition or the keys to preventing a chronic condition I generally focus on 7 things:Eat real food (produce, organic if you can, nothing processed or from factories)Get your nutrients (we all must supplement vitamin D and likely other nutrients)Reduce Stress (this might have to include turning off the news and your smart phone alerts)Get enough exerciseFix your biome (internal environmental diversity)Surround yourself with nurturing people who support your passions and healingGet enough SLEEP (Restorative good sleep!)  
So how much sleep is right for you? While this can certainly vary from person to person, and we need more during times of growth or stress, there are some guidelines.
 
In my scan of important articles (something I do every day) I stumbled on three – all addressing the sleep issue based on studies presented yesterday at the European Society of Cardiology.
 
The PESA study enrolled 3,974 healthy middle-aged adults and carefully tracked sleep quality and quantity. 
 
Here is what they found:Those getting less than 6 hours a night had significantly more atherosclerosis than those who got seven to eight hours.Those with the most fragmented sleep were more likely to have atherosclerosis (hardening of the arteries).Those with short or disrupted sleep were also more likely to have metabolic syndrome, (the combination of diabetes, high blood pressure, and obesity).  
The second study presented that in 1993, 50% of all men born in 1943 and living in Gothenburg were randomly selected to participate in a study. Of the 1,463 invited, 798 (55%) men agreed to take part. These men were followed for 21 years. Here is what they found:

Sleeping five hours or less a night, doubled the risk of cardiovascular disease compared to getting 7 or 8 hours of sleep.The magnitude of increased cardiovascular risk associated with insufficient sleep was similar to that of smoking or having diabetes at age 50.  
The third study was a meta-analysis of 11 prospective studies of more than one million adults without cardiovascular disease published within the last five years. The researchers found:

Six to eight hours sleep a night is most beneficial for heart health. More or less is detrimental.  
So how do we make sure we are getting enough, quality sleep?
 
A regular routine of going to bed at about the same time each night is important.  The 7 keys to health that I started with at the top of this article are important!
Avoid alcohol and caffeine before bed, and avoid large meals or vigorous exercise right before bed.  Having the room dark, cool and avoiding the blue lights of screens is important.
 
One important tool, I use, just hitting the market for the first time to monitor your sleep for both quality and duration is the Oura ring. I have found the data so helpful as I now know what my challenges are. The Oura ring tracks total sleep time, time in bed, resting heart rate and how much time you are awake, in REM, light or deep sleep. The app then makes recommendations for you based on your own data! This ring also tracks heart rate variability, a key measure of health, as well as your steps and calories burned.

I find brain science fascinating and I’ll admit I’m a chemistry geek. Even if you’re not, you may be interested in what drugs, alcohol, and even behavioral addictions can do to you and how addiction can affect your brain.
 
I run an outpatient addiction clinic based in Portland called Fair Start. At Fair Start we help addicts—mostly young adults—recover from drug addiction.
 
The majority of our patients are addicted heroin and other opioids. Often they are using several drugs at once, including meth.

Most people know me as a medical doctor. Most people don’t know that I’m also a recovering alcoholic.
 
Or that my wife, Maiya, is a recovering opioid addict.
 
While I’ve been in active recovery for 15 years now and I’m not shy about sharing my story, I’m about to go public with all of this in a big way.

So how does addiction affect the brain? No child is born thinking, “I want to grow up to be an addict or alcoholic.” 
​Even when our using or drinking starts having a significant negative impact on our lives, most of us will deny that a problem even exists. That is one baffling aspects of addiction.
 
Addiction hijacks a person’s normal common sense.  To the outsider, it seems we have no willpower.
 
But that’s not what’s actually going on.
 
We now know where most of the substances we misuse act in the brain. What happens in our brains when we experience pleasure? “My dopamine made me do it,” a phrase my colleague and friend Marvin Seppala, a medical doctor who is also the Chief Medical Officer of Hazelton-Betty Ford, often says.
 
“My dopamine made me do it” is closer to the truth than you might think. Dopamine is a neurotransmitter responsible for the sensation of pleasure.
 
When we eat, drink water, or make love there is a small release of dopamine. The rewarding nature of this feeling of pleasure has us returning to eat, drink, or make love. These natural activities sustain our lives.
 
Drugs and alcohol bathe our brains in dopamine, which makes us feel good.

But when we overuse them, our brains get flooded with too much dopamine. The rewarding feeling is so intense that we want that feeling to continue. If we have a tendency towards addiction, our forebrain brain—the part of the brain that controls and moderates our impulse to stop before we hurt ourselves—gets overwhelmed.

Instead of helping us stop, our addicted brain does the opposite: we get intense cravings for more drugs or alcohol, we can think of nothing but the next fix or the next drink. We feel we’ll die without more.
 
I’m simplifying—there are more neurotransmitters involved than just dopamine—but dopamine is one of the keys.
 
When rats are given the opportunity to self-administer an electrical stimulation to their brains that releases dopamine to levels seen with cocaine, some will continue to do this without eating or drinking water until they die.

Some humans do the same: binge drinking or tweaking on a meth run that can last until the person collapses in exhaustion or even dies.
 
I was a binge drinker.

I couldn’t have just one glass.

​Once I started drinking I had no off switch. I needed another drink. Addiction affects your brain. But have you ever wondered how? Addiction Spectrum - Dr Paull Approved Addictive substances affect the release and inhibition of dopamine by imitating neurotransmitters and activating receptors in key areas of the brain.
 
Alcohol, opioids, and even cannabis sometimes can suppress the inhibitory action of the GABA neurons, causing an unregulated release of dopamine.
 
Amphetamines and nicotine directly activate the release of dopamine into the brain.
 
Cocaine works by blocking the process through which dopamine is removed from synapses, resulting in a continued state of euphoria.
 
Isn’t it interesting how all these substances actually work on the same receptors, activate the same chemicals, and hence have the potential to produce the same addictive effects?
 
The problem is not that addicts are seeking pleasure.
 
The problem is that when you take the desire for dopamine too far, some people can’t stop.
 
Now that you have a better understanding of how addiction affects the brain, what’s the solution?
 
In order to heal from addiction, you have to heal your brain.
 
You have to find non-addictive ways to feel good.
 
You have to avoid addictive substances that overwhelm your brain with dopamine and replace unhealthy addiction with healthy behavior.

There is a way forward out of addiction. No addict is “hopeless.”
 
We can end the addiction epidemic.

Success generally doesn't just bite us in the butt as we sit around waiting for it.  Luck favors the prepared, and my wish for us all is that we move forward in 2017 with purpose. Set your sights on worthy goals, but don't forget to enjoy the journey.

I thought I would share my personal goals with you in case it might motivate or help someone gain clarity on their own goals and journey.

by  Paul Thomas, M.D. Picture Bernie talked of revolution, Trump talked of revolution. Too bad we couldn't combine the best each had. But the real revolution is just getting started. Regardless of party, religion or any other group that would separate us, we the people are waking up.  We are realizing that mainstream media is just a phony puppet front for big business and their interests. Powerful interests who own the media have intentionally kept vital information from us, and fed us a steady diet of lies and misrepresentations.
Where is the real news? We are starting to realize it is with us already, yes indeed, on the Internet, social media, and even in print. We are waking up to the fact that we must question everything.  If your news media or the CDC or a vaccine manufacturer says this vaccine is safe, then they put millions into promoting it ... THINK again. No vaccine is completely safe, so what are the real numbers?  Where are the real safety studies?
What?  The don't exist?  How can that be? Across the country Integrative Physicians, Functional Medicine doctors, Naturopaths, Chiropractors and many other healers in the "alternative medicine" world are starting to speak up. Starting to wake up to the disaster that has been shoved down our throats that is the CDC vaccine schedule, that is pesticides and herbicides, that is GMO foods, aspartame, acetaminophen and a drug for every symptom you can imagine, syndromes made up of a group of symptoms - all for the purpose of selling more drugs. If there was a large pediatric practice with over 13,000 patients, where patients had been empowered with all the facts and the ability to make informed decisions about which vaccines to do when (INFORMED CONSENT) and then the data was collected over many years that showed no vaccines or a slower careful vaccine schedule could virtually eliminate autism, significantly reduce illnesses, office visits and ER visits and reduce developmental delays ... would you think your news outlets would jump all over that? If this experience and data were submitted for publication, you would think journals would be fighting to get the chance to publish such society saving information.  Think again. Time and time again, journals who publish anything that has to do with vaccines and autism are threatened by the big powers that be and told to retract the study (if it's already printed) or forced not to print it (if not yet printed). You won't hear about this in the news on the boob tube, the papers or any other mainstream outlet. You won't hear about this from your doctor, unless you have already left your doctor for one more enlightened. Just this week JAMA just published a study showing the flu shot was safe for pregnancy! [http://jamanetwork.com/journals/jamapediatrics/fullarticle/2587559] The news of this wonderful finding is being blasted all over mainstream media. Just a quick look at the data, what they actually found and guess what? There will be 4 more cases of autism for every 1000 women who get the flu shot in the first trimester.  We have about 4 million births a year in the USA so that would amount to an additional 16,000 cases of autism a year.  It should be a crime to give the flu shot during pregnancy. Be careful where you get your news.
The revolution is under way.
Mainstream media - I can't hear you anymore. I'm tired of your lies. Your inability to investigate the real story shows that those who pay your salary have you under complete control - you are now obsolete.
