I'm not sure about "The Mists of Avalon" and "Lolita". Sure the incestual scene in the Mists is central but it isn't the main theme in my eyes. The main theme is a world going to be lost.
As for "Lolita" - there it is'nt really incest, because Lolita and HH aren't really blood relatives. For me, it is more about pedophily.

Where is the incest in Vladimir Nabokov's Lolita (#7)? She is under age, but he is not a blood relation of her, so in my book there's no incest... (And two of the voters didn't rate it, neither have they marked the book as 'read', which makes me wonder...).

Another book that makes me wonder, and I've read it more than once, is Jane Austen's Pride and Prejudice (#48). Incest? Where? And a main theme at that... Tsk... Seems I really need to reread it again.

Well, he's her stepfather but that doesn't make him a blood relation... I'm aware different countries have different rules, but still... Incest? There are a few other titles on this list I have my doubts about but as it is a long time ago, I'm not 100% sure [99.9% though :-)]

I noticed another one of which I'm pretty sure incest is not present, let alone it being the main theme of the book: Harry Potter and the Sorcerer's Stone (#108).

Again, can the voter please enlighten me, because I really, really seem to have missed something...

Gone with the Wind? The only possible example is Ashley and Melanie being cousins, but it's hardly a "major theme".

To Kill a Mockingbird? Yes, there's a brief indication that Mayella Ewell was abused by her father ("what her Pa do to her don't count") but again it's not a major theme.

I guess in "Frankenstein" Victor is technically Elizabeth's brother.. but as a central theme? plus, they're not related by blood?

Can I request that my books, Quid Pro Quo and Sine Qua Non, be removed from this list? The relationship is between two stepsiblings and they are not blood-related.
